Rehabilitation:

Our team of skilled physical, speech and occupational therapists cater to your individual needs with the underlying goal of returning you to your highest level of health and wel-being. We offer the Wii Therapy System so residents can have energetic fun while working on balance, hand/eye coordination and mobility. Programs such as these are ideal for patients recovering from surgery, joint replacement, stroke, injuries and other partial or temporary disabling conditions.

Our world-class team of caregivers is dedicated to providing individualized care and services to thoses residents who choose to make our home - their home. Cusine

Our culinary team puts heart and soul into the preparation of both tasty and nutritious food. Our motto is: Just like home. This means we are able to modify menus to accomodate residents' cultural and ethnic diversity - in an effort to satisfy a broad range of tastes. Additionally, residents are able to choose between various dishes on the menu as well as having the ability to request some of their favorite recipies from home which allows simple happiness that comes from a meal that tastes homemade.

Step by Step

1. Prior Admission

The process of admission to any facility can be overwhelming. If you desire, one or more members of our friendly team of professionals will meet you at the hospital of your home to address any concerns you may have about your care or that of your loved one. We will also provide you with a list of items to bring to the facility.

2. Admission

Our admissions Coordinator will sit with you an/or your responsible party to fill out the required forms and discuss the payment options available to you. To aid inthe process, we kindly ask that you bring copies of all your insurance cards.

3. Plan of Care

During your stay, the interdisciplinary team will meet to discuss the individualized plan of care for you or your loved one. You and/or your family are always invited to attend these meetings, which address both physical and emotional needs. Progress reports can be provided to personal physicians to ensure continuity of care.

4. Discharge Planning

Upon discharge, one of our professional social workers will assist you with a safe transition to your home environment. Our assisted living facility on our campus is available to provide you with a place where you may live independently or stay while you regain your independence after your hospital stay.

Admissions accepted 24 hours a day, 7 days a week. Medicare/medicaid Certified.
